Patient rights and responsibilities

Last updated: October 27, 2026

Your rights: to know the name, credential and license state of the provider treating you (given in your care materials before treatment begins); to a clear explanation of risks, benefits and alternatives before you decide; to refuse or stop treatment; to ask questions and get answers from a person; to see and get a copy of your record and ask for corrections; to privacy (your health information is never sold); to know the price, billing interval and cancellation terms before you pay; to in-person care or a second opinion; to ask for a different provider; to care without discrimination; to free language help; and to complain without any effect on your care.

Your responsibilities: give accurate and complete information, tell your provider where you are, follow your plan or tell your provider if you stop, report side effects, keep your contact and order details private, and pay for services you agreed to.
